Billing & Insurance Information

Health insurance and surgical billing can be complex, and coverage varies by plan. Patients are responsible for understanding their insurance benefits, including network participation, deductibles, co-pays, and any coverage exclusions.

Before surgery, contact your insurance company to:

• Confirm Boulder Surgery Center is in network
• Verify your plan includes benefits for procedures at a “freestanding ambulatory surgery center”.

Boulder Surgery Center Estimate

Estimates: You will receive an estimate via text/email from “Clariti” prior to your surgery. If you have questions about your estimate, please call before your surgery to avoid delays on the day of surgery.

Payment Requirements: 75% of your estimate is required prior to your surgery – this can be paid online (ePAY Healthcare- Make Payment) or at check-in.

Payment Plans: We offer financing through CareCredit, these need to be set up prior to your surgery. Care Credit allows you to choose payment terms that work for you. No interest is charged when the balance is paid within the selected promotional period (such as 3, 6, 12, or 24 months).

Billing / Insurance: What Charges to Anticipate

You will receive separate bills from providers involved in your surgery:

1. Boulder Surgery Center – Facility Fee:Due by the day of surgery and payable at check-in. An estimate is sent by text and/or email before your procedure.
2. Surgeon’s Office – Physician Fee:Billed before your procedure. Contact your surgeon’s office with questions about your estimate.
3. Anesthesia Provider – Anesthesia Fee: Billed after your procedure by Boulder Valley Anesthesia. For questions, call 303-415-2532.
4. Surgical Assistant: If your surgeon uses an external surgical assistant, a separate bill is issued after your procedure. Contact your surgeon’s office with questions.

Accepted Forms of Payment

We accept:

• Cash – must be for the exact amount. We are unable to provide change.
• Credit Cards – all major debit/credit cards
• Personal checks up to $500

Payments over $500 must be made with a cashier’s check or money order.

Out-of-Network Plans

If Boulder Surgery Center does not participate in your insurance plan, you may still receive services at our facility. However, your insurance company will consider these services out-of-network, and you may be responsible for a larger portion of the bill.

Self-Pay Patients

Self-pay patients are required to pay in full prior to their scheduled service.

Preparing for Your Visit

Please bring the following items with you on the day of your procedure:

• Payment for your estimate 
• Your insurance card(s)
• A valid driver’s license or state ID
